Faculty Request Forms

Submit requests on paper forms, electronically, or by FAX to (413) 577-0037. Please provide complete and accurate bibliographic citations for processing.

As the shelving space for Print Reserves has been decreased, and student demand that articles and book chapters be available on electronic reserve as increased, starting with Spring 2009 semester, all articles and book chapters that were circulated in manila folders will no longer be accepted for print reserve, instead, they should be submitted to electronic reserves.

Electronic Reserves processing forms:
On the Electronic Reserves page, choose "ERESERVES PROCESSING FORMS" to display the list of available forms. There are separate forms for Book Chapters and Journal Articles. Use the MS Word forms and save them as files on your computer for repeated use.	MS Word, HMTL, PDF	MS Word, HMTL, PDF
