Proxy Card Application

What is a proxy?

UMass faculty, staff and graduate students (and undergraduate students registered with Disability Services) are eligible to designate and authorize another individual as their proxy for borrowing. Graduate students may designate ONE individual as their proxy for up to one year.

Upon receipt of a proxy card application, a proxy card is issued to allow the proxy to request and check out materials for the Patron of Record. The Patron of Record is responsible for all materials charged out on the proxy card.

In order for a proxy to pick up items on hold for the Patron of Record, the items must be requested using the proxy's library barcode, found on the back of the proxy card.

All current UMass Amherst faculty,staff and graduate students are eligible for a Library proxy card. Students who are currently registered with Disability Services will need to provide a letter of authorization from Disability Services requesting Library Proxy card eligibility.

Personal Agreement: I agree that I will be responsible for all materials charged out on the proxy card and I will respond to all correspondence concerning charges on this card (e.g. overdue notices).

Lost or stolen cards should be reported immediately. Pick up proxy card at Du Bois Library Circulation/Reserves Desk
