Borrower's Card Application
Massachusetts residents and UMass Amherst alumni - complete this form if you are not a UMass or 5 College Faculty, student or staff member. The process will be completed when you arrive in person to pick up your card. We require a current valid state-issued photo ID and may require additional documentation for identity and address verification purposes.The card permits borrowing of print and media items from the UMass-Amherst Libraries physical collections.
For Massachusetts residents seeking online access to eBooks and other electronic resources, the Boston Public Library offers access via the BPL eCard:

Boston Public Library eCard:
Massachusetts residents can obtain online access to eBooks and other electronic resources thoruhg the Boston Public Library via the BPL eCard.
The BPL eCard is a Boston Public Library card that you can sign up for online which provides access to all online resources, including OverDrive and Hoopla. The BPL eCard is available to anyone who lives, resides part-time to attend school, owns property, or works in Massachusetts.
Proxy Card Application
For UMass faculty, staff and graduate students.
UMass faculty and staff members, graduate students (and any student registered with Disability Services) are eligible to designate and authorize another individual as their proxy for borrowing. Graduate students may designate ONE individual as their proxy for up to one year. A proxy card is then issued to allow the proxy to request and check out materials for them. The individual who has authorized the proxy is responsible for all materials charged out on the proxy card.
The UMass Amherst Libraries are committed to protecting each library user's privacy and confidentiality by keeping any personally identifiable information about users private.
Library Carrel Application
Applications for annual academic year assignments are accepted between April 1 and April 30 from UMass Amherst Grad Students, faculty and authorized Visiting Scholars. Assignments are made on a prioritized first come, first served basis.
